The University of Arizona requires verification of measles and mumps immunity for all students born after 12/31/56. If you were born prior to 12/31/56 you are exempt. Immunization records must be obtained from your health care provider indicating dates of:
a) Two (2) MMR immunizations, at least one of which must have been administered after 12/31/79.
b) The first vaccination is given at or after 1 year of age and the second is commonly given at or after 4 - 6 years of age. If your records do not reflect this immunization schedule, the two MMR vaccinations must be at least 28 days apart from each administrated date.
c) A lab test that shows immunity to Measles, Mumps, and Rubella.
You will not be permitted to register for classes without proof of immunity against Measles and Mumps on file with the Campus Health Service.
When your record is received and processed, your hold from UAccess will be removed and your Next Steps Center (undergrad) will have a green check mark. Please allow 1-2 business days for processing.
You may download the New Student Immunization Requirement form and have your health care provider complete and sign it, or you may fill in your personal information and attach a copy of your vaccination/blood testing records.
If you have an electronic copy of your immunizations, you can submit the record through your PatientLink portal by clicking the icon below and signing in with your NetID and password. On the homepage you will need to select the Forms tab and then Immunization History. From there you will be prompted to enter your immunization dates and attach your documentation. Please allow 2 business days for your documents to be processed and for the immunization hold to be lifted from your account.
